Shift:    hours will vary, and will have to work some weekends

Qualifications

• A Bachelor's degree with a major is vocational rehab, psychology, social work, counseling, or a related human services field. Master's degree preferred.
• At least 1 (one) year of full-time experience working with persons with severe and persistent mental illness (SPMI) required.
• Prior vocational rehabilitation experience preferred.
• Assertive community treatment experience working in off-site community settings consistent with the PACT model of service delivery preferred.

Description

• Assesses, plans, develops, coordinates and provides treatment, rehabilitation and support services to persons with severe and persistent mental illness.

• Provides crisis intervention.

• Serves as client advocate and liaison by providing outreach and supportive services to clients.

• Makes appropriate referrals and consultations both within and outside the agency.
