Qualifications

• Bachelor's degree from accredited university or college with a major in counseling, social work, psychology,or other human services field 
• and
• One (1) year of full-time experience working with severely and persistently mentally ill adults;

• Current and valid driver's license with no more than six (6) points.

Description

• Conducts assessments and reassessments of assigned clients in a timely manner.
• Conducts home visits prior to completion of the intake assessment process to evaluate the safety and well being of the client in the setting the client resides.
• Coordinates development of individualized integrated service plans in partnership with the clients, the client's guardian (if applicable) and the treatment team.
• Develops service plans that include measurable goals and objectives derived from the client's assessment and identified time frames for achievement of goals.
• Updates service plans at least every 6 months or at any time significant changes occur in the client's condition, situation, or circumstances.
• Facilitates referrals to specialists and/or additional healthcare resources.
• Assists clients in gaining access to needed financial and insurance benefits, employment, medical, social, education, and other services.
• Works with the client and the client's natural support system to develop and implement the client's service plan.
• Follows-up on services to determine the effectiveness of the service plan in meeting the client's needs.
